²ÝÝ®ÊÓƵ operates thousands of facilities where we provide life-saving treatment for our patients. To best distinguish between our facilities, which span across the United States and other countries, we use a facility number to identify each site (e.g., Facility # 00387). For additional clarity, we encourage you to include the city and state where your Leased space is located along with the facility number in your communications (e.g., facility #00387.100, Baltimore, MD). Your Lease may have the facility number included in the notice address section if the facility number was assigned prior to Lease execution. Facility numbers can also be found on most ²ÝÝ®ÊÓƵ communications to landlords, including rent checks, estoppel certificates and legal notices that we send once the facility has opened. Using the facility number in all correspondence with us will help ensure that the property is properly identified.

Comply with the notice provision in your Lease regarding notice; however, please note that if your lease was entered into prior to 2012, to ensure the timeliest response from ²ÝÝ®ÊÓƵ, also send any demand, notice or declaration in writing in one or all of the following ways:

Respond in writing to the ²ÝÝ®ÊÓƵ Legal Notice Address and to the party/address indicated on the default notice.

Reporting a Physical Issue to ²ÝÝ®ÊÓƵ

In case of an emergency, at or around the facility (a roof leak, plumbing problem, snow removal request, etc.), please follow the terms and notice provisions of your Lease. If you have questions, email RED@davita.com.

In all cases regarding issues in the facility, such as business practices, hours of operations, etc., please reach out to the ²ÝÝ®ÊÓƵ Facility Administrator (FA) at the facility to notify them of the issue and refer to the terms of your Lease regarding repairs.
